Appointments:

At Champlain Gynecology, we look forward to our meaningful partnership starting on your first visit with us! To schedule an appointment, speak with our front desk staff at Champlain Gynecology by calling (802) 879-1802 during operating hours.

We require at least 24 hours’ notice to cancel or reschedule your appointment. You can do this by calling the office during the operating hours above. Failure to do so may incur a no-show fee ($100) that will be charged to your credit card on file. The doctors also reserve the right to reschedule your appointment if you arrive more than 10 minutes late. If you have an extenuating circumstance for your absence, please reach out to the office as soon as possible. We will do our best to make exceptions for emergencies. Multiple no-show fees or late arrivals may results in a warning letter or referral to a different practice.

After scheduling your appointment, you can download and print the Patient Health History Form to bring with you on your first visit. Please complete these forms before you arrive to ensure a prompt check-in at the doctor’s office.

Release of Medical Records:

  • Most patients only need a portion of their medical record, but we can provide a full copy if necessary. When completing the Authorization for Release of Medical Records form, please specify exactly what information you need to make it easier for our staff to complete your request.
  • Copy fees: $5 flat fee for the first 10 pages. $0.50 per page for each additional page. To reduce costs, consider requesting only the past 2–5 years of records and other high-level information specific to your request. Currently, we only accept cash or check for medical records.
  • No charge for records requested by an insurance company, healthcare provider, or facility for continuing medical care, such as a primary care provider.
